Our Health Information Management (HIM) team — Christina, Gayle, Julie and Annette — is located in the Medical Records Office at our Brooks Hospital campus. Each demonstrates Brooks’ I-CARE Values of Integrity, Compassion, Accountability, Respect and Excellence, as part of her daily interactions at all levels within the hospital.

Our Health Information Management team is ready to assist you in managing your medical records, whether you want: copies for your own record,  to transfer records to a provider, allow access to your records by another person, establish health care proxy, access your record online through the Brooks Memorial Hospital Patient Portal, or need assistance with other services related to your medical record with us.

Birth Certificates

Birth certificate information is filed by the hospital with Vital Statistics. To get a copy of a birth certificate, please contact the Dunkirk City Clerk at:
