Lake Erie Regional Health System of New York (LERHSNY), our mission is to provide high quality health care where it matters most – right in your own communities. With a complement of established, full-service hospitals to a wide network of clinics and medical offices, be assured that your health and the care of your family is our top priority.
LERHSNY was formed in 2008 as the parent organization for Brooks Memorial Hospital in Dunkirk and TLC Health Network comprised of Lake Shore Health Care Center in Irving and Gowanda Urgent Care & Medical Center in Gowanda. Another TLC Health Network facility, Tri-County Memorial Hospital in Perrysburg, was closed due to flooding in 2009.
As part of TLC Health Network, primary care and family medicine services are provided at Conewango Valley Medical Center and in Forestville. A new facility, Tri-County Medical Office, will relocate in Forestville in summer 2012.
Chemical dependency rehabilitation clients receive services at Cassadaga Chemical Dependency Clinic and Derby Chemical Dependency Clinic through TLC Health Network and outpatient care for behavioral health patients is available through TLC’s Assertive Community Treatment program in Fredonia.
When you choose to trust your care to the staff at any of LERHSNY’S locations, you can trust that you’ll be offered an array of health services in a network which encompasses emergency, obstetrics, primary care, diagnostic imaging, orthopedic and arthroscopic surgery, radiation oncology, long term care and home health, dental, and child and adult day care services.
