Q. How do I get a copy of my Birth Certificate?

A. Brooks Memorial Hospital does not have copies of Birth Certificates, but the City of Dunkirk does. You will need to contact Dunkirk City Clerk to get an official copy of your birth certificate. They will charge you $10 for this, but you can do it via mail. Write a letter to them requesting the certificate, and have your signature on the bottom of the letter notarized. They also need a photocopy of your drivers license.
